Stealth Infections May Be Speeding Up Your Aging Process – Here's How to Push Back

It should have escaped the attention of exactly none of my regular readers that in recent years I’ve become increasingly fascinated with the aging process. Granted, I’ve joined the ‘senior’ ranks so there’s a natural interest, but more broadly, it’s the science that’s got me hooked. The amazing advances in the biology of aging over the past two decades have made it clear that aging is the primary driver of the diseases that limit and end most of our lives, everything from heart disease to cancer. This understanding is embedded in the so-called Hallmarks of Aging which break down the aging process into a number of distinct but interconnected biological processes.
Well, there’s a new wrinkle (excuse the expression) to this story. Most of us probably think of aging as something that happens exclusively inside of us – our parts wear out or sometimes they even turn against us. But increasingly, scientists are assembling evidence that the aging process is profoundly affected by our exposure to the outside world. We’re talking “pathogens” -- viruses, bacteria, fungi, parasites, all the things from the outside that enter our insides, in the air we breathe, the water we drink, the food we eat. And the nature of that outer-inner interaction affects, all too often weaponizes, the Hallmarks which, even without any outside assistance, can bring on frailty in our later years.
There are a lot of compelling theories about the aging acceleration effect of living in a world full of pathogens, fewer about what to do about it, although some practical strategies are emerging. So, what’s happening on the frontlines of the biology of aging – and what to do about it? Here’s my topline on this fascinating stuff:
The pathogen paradigm shift.
The old take on pathogens went something like this: you get an infection; and then, with or without the help of antibiotics, your immune system knocks it out and you move on. The new paradigm was laid out in a December 2025 paper in Aging Research Reviews by a duo of researchers from Mount Sinai and Mass General with a report titled, “Pathogens accelerate features of human aging: A review of molecular mechanisms.” This highly technical article says, in so many words, ‘wait a minute, it’s not that simple.’ Pathogens worm their way into the basic machinery of the body: your immune system, your metabolism, even the epigenetic system that governs the expression of your genes. Meanwhile, you may not even be aware that you’ve got a toxic bad actor inside you compromising your health and longevity – almost certainly, an assortment of them. You may not have any obvious symptoms. No matter, the bad bugs are aging you.
The “inflammaging” angle.
Aging, and the diseases that aging gives rise to, is, to a large degree, driven by inflammation. That’s why researchers like to talk about “inflammaging.” They’re not talking about acute inflammation, the body’s first and necessary response to say a sprained ankle, but rather, chronic inflammation that keeps the immune system in a constant state of low-grade activation. A handful of common viruses like cytomegalovirus (CMV), Epstein-Barr and the herpes viruses are usual suspects, viruses that can lie dormant for decades, as they drain the resources of the immune system. Here’s an example from the bacterial world: too many of us suffer from low-grade tooth and/or gum infections which, like the viruses, can generate inflammation capable of taking root in vulnerable places, like the heart. Inflammation “arms” the LDL cholesterol in the lining of our coronary arteries, setting in motion a process whose end-result is the atherosclerotic plaque that causes heart attacks.
Infection — and the exhausted immune system.
Remember however, the immune system isn’t just in the business of fighting viruses and bacteria. It’s also responsible for clearing out damaged cells or recycling them for parts (autophagy). When the immune cells themselves begin to decline in number and potency, exhausted from a lifetime of countering pathogenic threats, then the metabolic trash begins to pile up, another source of inflammation.
Making matters worse, cells allowed to grow old and dysfunctional by a now less than vigilant immune system stop doing their appointed jobs and start generating inflammation themselves, what researchers call SASP or the senescence-associated secretory phenotype. Oxidative stress (think free radicals) increases as these senescent cells pile up in our blood vessels, our joints (think osteoarthritis) and our skin (say no more), doing more than their share to make us look and feel old. Basically, it's one vicious cycle after another, with the effect of depleting our immunity and driving inflammation in a myriad of different ways. We can see the result in studies of elders. Their inflammation markers are high and yet their immune systems can barely mount a response to a vaccine. It’s like their immune systems are permanently stuck in idle, unable to respond to new and potentially lethal threats like pneumonia.
Clogged fuel line: mitochondria and infection.
The mitochondria are the cells’ powerplants, where glucose and fatty acids get combusted to make the fuel that runs the body. Aging is hard on the mitochondria – they decline in number and efficiency and over time generate more oxidative stress and become increasingly prone to mutations that can contribute to the development of disease. Pathogens, viruses especially, make everything worse. Viruses essentially hijack the body’s power system so they can survive and reproduce. Scientists now recognize mitochondrial dysfunction as one of the major engines of aging. Exactly how much pathogens and their infections contribute to this dysfunction isn’t as well mapped out as, for instance, the infection/chronic inflammation connection, but the theoretical blueprint is being worked on.
The brain: where pathogens come home to roost
For years, cardiologists have warned their patients that an untreated tooth/gum infection could increase their risk of heart disease risk. And for some time, I, and my integrative physician peers, have emphasized that a disordered gut microbiome, thrown off by infection, as well as by poor diet, can allow pathogens to leak into the bloodstream, triggering inflammation most anywhere in the body.
But the brain is a new frontier in this infection-inflammation-aging research. An eye-opening study came out last year that made a convincing case that taking the Shingrix vaccine that protects against shingles (caused by a herpes virus) was strongly associated with a lower dementia risk. The logic is: not getting shingles, and for that matter, not having “subclinical” activation of the latent virus (remember, herpes viruses like to hide, often causing no symptoms), means less “neuroinflammation” and an immune system better able to clean up metabolic waste in the brain. That includes the amyloid plaques that are strongly associated with Alzheimer’s disease. More clues linking pathogens with neurodegenerative diseases? We see evidence of persistent pathogen activity in the brain tissue of people who have died from Alzheimer’s and Parkinson’s.
Running out of reinforcements?
The aging body depends on a continuous supply of new parts, especially the immune system which has to generate billions of new cells daily to do its job. As we’ve discussed, an immune system constantly responding to chronic infections like herpes can begin to run out of gas. Not only do these infections increase the demand on the system, but the inflammation and oxidative stress generated by these viruses can damage the stem cell niches where the new cells are produced (in the case of the immune system, in the bone marrow), interrupting the supply chain of reinforcements.
Deep dive: pathogens at the genetic level.
Pathogens may well be working against our longevity interests at the deepest molecular and genetic levels. A steady diet of inflammation and oxidative stress can alter our epigenetics, that is, the chemical markers that control gene regulation – which genes get turned on and when. Some of these changes amplify the harmful effects of the Hallmarks of Aging, like chronic inflammation and mitochondrial dysfunction. (“Biological age” tests measure epigenetic patterns so if you take one and get back a number older than your chronological age, that’s what’s going on.) So, the authors of the Aging Research Reviews paper argue, since toxic viruses and bacteria make the destructive Hallmarks that much worse, it’s only a short jump to conclude that these pathogens help reconfigure our epigenome, again for the worse. It’s a chilling hypothesis.
Pushing back on pesky pathogens.
The challenge is to develop more sophisticated diagnostic tests that can identify pathogens and the harm they do in the absence of active infection and visible symptoms. That’s work in progress. In the meantime, there are therapeutics that can push back against inflammation and low-grade infection, most of which I’d put in the “not ready for prime time” category because we don’t yet have adequate low-term efficacy and safety data. I’d put the pharma drug rapamycin (at low doses) and the herbal supplement Cat’s claw also in that category. Ditto so-called senolytic therapies like the supplements quercetin and fisetin which have been shown to help the body clear senescent cells. They’re promising but we still don’t have a good handle on when they’re appropriate and for whom and in what dose. One supplement I am willing to consider is methylene blue, a chemical that’s been in medical use for over a century and has antioxidant and energy-boosting properties. It’s available over the counter but it should be taken under the supervision of a knowledgeable health care provider, as there are side-effects and contraindications.
Common-sense measures are your best defense.
You know the old saw about an ounce of prevention being worth a pound of cure. Granted, we all have different risk tolerances. But certainly consider common sense public health measures to avoid being infected with nasty pathogens in the first place. There are loads of ways to push back on pathogens, but these would be my top five (no-brainers) pathogen preventers. They’re simple and sensible – and can go a long way keeping your body protected: mask-wearing in crowed spaces, up-to-date vaccinations, frequent hand washing and investing in a good home air filter. And don’t forget to visit your dentist regularly, to nip any mouth infections in the bud.
